The Directed Blood Donation Patient Consent form is required to be filled out and signed and witnessed before the Blood center will schedule designated Blood donations. The form lists those whom the individual receiving the blood wishes to nominate as potential donor candidates. Further the form names the recipient of the donated Blood and is to certify that the individual receiving the Blood understands certain things that are outlined on both sides of the the form.
